Search Content on a Specific Page

I would like to use the plugin on one specific page so that members can search only for content that appears on that page. The page contains text, links, and images. I tried to customize the form in a manner I thought would create this function, but when I add the form to the page, I get “Nothing found” results, no matter what search term I use. See the attached screenshot; you’ll see I entered the term “annual” in the search. You’ll also notice that the very first document listed on the page is an Annual Report. Yet, the search does not recognize it. I’m sure the issue stems from something I’m doing incorrectly. Please advise how to get the function I’m looking for. Thanks.
