It is not media but the featured images displayed in the search results of your website.
You can hide them by adding the below CSS code in the Custom CSS option of Ivory Search plugin settings on the path Admin Area -> Ivory Search -> Settings -> Settings

No, it is not possible without developing custom code.
It seems the TOC is generated dynamically therefore the plugin can’t find it.

Yes, you can achieve this by enabling the search form option as shown in the attached screenshot.

The category search option only works for the post categories and not for custom categories.
