You can hide it by adding the below CSS code in the Custom CSS option of Ivory Search plugin settings on the path Ivory Search -> Settings -> Settings

If you still don’t understand it then please deactivate the Ivory Search plugin and see whether you see them in the search results page.
I visited your shared website but didn’t see the search form having ID 3750 on your website.
Could you please share the screenshot and the exact page URL where you are using it?
It displays fine for me on the below page as shown in the attached screenshot.
Could you please test it using a different browser?
